Request for Proposals - Registered Nurse
 
Garrett County Community Action Committee, Inc. has an incredible 12–18-month part-time opportunity for a contractual Registered Nurse (RN) available in Garrett County, Maryland.

Garrett County Community Action has a powerful purpose – “Empowering people by removing barriers and offering pathway opportunities to achieve aspirational goals through advocacy and services.” To enable this purpose, Community Action has several existing and new initiatives designed to provide equal opportunity and access to meet the unique needs of the individuals we serve.
 
Through a grant-funded initiative serving older adults in Garrett County, Community Action delivers services to reduce the impact of functional impairment and disability, enhance motivation and self-efficacy, reduce health disparities, improve emotional health, and reduce hospitalizations and nursing home days through the Community Aging in Place-Advancing Better Living for Elders (CAPABLE) program.
 
Following the completion of required CAPABLE training through Johns Hopkins University School of Nursing—five 60-minute modules, online and self-paced—the Registered Nurse (RN) will collaborate with 35 older adults in their homes to deliver nursing components of the CAPABLE program. The CAPABLE program consists of up to four home visits over 4-5 months. We are looking for registered nurses who:

• Lead with heart – display empathy and compassion for participants to build a foundation for action plan development and implementation through a comprehensive interview process
• Engage participants to identify behavioral and environmental challenges that may impact function, problem-solve, coach, and guide them to build self-efficacy and confidence
• Communicate with the CAPABLE Occupational Therapist (OT) and other team members to ensure close collaboration throughout and after the program
• Possess strong clinical skills to document, educate, and provide resources to address the participant’s future needs effectively to promote the ability to live at home and in the community
• Ideally, you’ll have experience as an in-home care provider, possess a passion for developing and implementing client-driven action plans, and stay motivated to improve the lives of CAPABLE participants

Qualifications: Individuals must be a registered nurse licensed by the state in which she/he is practicing. Ability to work from a collaborative, client-directed model. Willingness and ability to implement a protocol and balance with clinical judgment.
 
Preferred Qualifications: Bachelor's degree in nursing and experience in-home care preferred.

In addition to the fee per client, mileage expenses will be reimbursed at the current IRS standard mileage rate. This program allows for flexible scheduling and is good opportunity for supplemental income.

Family Service Coordinator and Classroom Intervention Content Specialist I

Garrett County Community Action is recruiting for a Garrett County Judy Center Family Service Coordinator/Content Specialist. The Judy Center is a state funded program whose staff help prepare children age birth through kindergarten for school readiness. The successful applicant must have practical knowledge and understanding of family services, coordinated case management, early childhood education and family engagement. He/she must be able to plan and coordinate academic and family services, support and promote parent engagement activities, have outstanding communication and computer skills and the ability to work in a confidential and professional manner when working with children, families and partners.
 
Key components of the position include:
o Assisting families in Pathway Goal Planning to help them identify needs and access services
o Coordinating services with staff and partners to ensure that families can support their child’s school readiness
o Planning and implementing parent-child activities and trainings
o Coordinating ongoing outreach activities to engage child care partners and eligible families with the Garrett County Judy Center
o Maintaining adequate data collection to monitor progress and track services, referrals, collaborations and academic achievement
o Planning and implementing academic intervention services in alignment with Maryland College and Career Readiness Standards and assessment data.
o Coordinating services with classroom teachers to enhance school readiness and academic achievement
 
Position Qualifications and Experience Requirements: Bachelor’s Degree in Early Childhood Education or related field is preferred. An Associate’s Degree in Early Childhood Education, or related field with 2 years of experience is acceptable.

Family Service Coordinator II
 
This position is responsible for working directly with the families of children enrolled in Head Start and Early Head Start programs throughout Garrett County, MD. The successful applicant must be able to broadly assess family needs and coordinate access to local services and programs. The successful candidate will:
 
• Coach families to create realistic goal plans, help achieve and track progress towards goals and make adjustments to plans as needed.
• Input a wide range of family data in the agency’s client database.
• Be responsible for understanding the range of services and programs available within the agency along with throughout the community and ensure clients are receiving eligible services to help them succeed.
• Work with classroom and school staff to understand the holistic needs of children and families enrolled in the program.
• Provide case management coordination with a team of specialists for children and families in crisis as needed.
 
Minimum Qualifications include: Must have a high school diploma or equivalency, with an AA degree preferred, in a field related to Social Work or Human Development.
The applicant must pass a criminal background check, possess a valid driver’s license and meet other pre-employment hiring criteria.
This is a permanent, full-time position offering a Monday-Friday, 8:00 am – 4:00 pm schedule with an hour paid lunch break. Some evenings may be required. Service Coordination Content Specialist II
 
Garrett County Community Action, Service Coordination Department is seeking a Content Specialist II for the Grantsville, MD office.
 
The Content Specialist II will be responsible for managing and monitoring the accuracy and completeness of electronic client files for all department programs. Program matter includes the energy assistance program and homeless programs. Responsibilities include coordinating customer access to available programs and resources by accessing family needs, coordinating client access to assigned programs, assist individuals gain information and access to health exchange information, provide case management coordination, serve on family centered teams, be familiar with the range of service programs available within the county. Must have ability to work with limited supervision. Experience working with the general public in a fast paced atmosphere preferred.
 
Qualifications/requirements
AA degree in related field or High School diploma with three years related experience in field. Demonstrated proficient computer skills including document management, client tracking, etc. Criminal background check is required. Must have a valid driver’s license and access to a vehicle for outreach.

Substitute Receptionist (Admin Support)

Garrett County Community Action Committee, Inc., is accepting applications for Substitute Admin Support Receptionist. This position is responsible for answering all incoming calls to the agency and connecting the caller to the correct department. Documents and scans all press information regarding the agency as well as scanning other agency documents as needed for electronic document management. Assists other departments as needed with typing. Helps visitors to the agency get to the correct area. Collect rent payments for Property Management Department.
 
Position Requirements: Excellent communication skills, ability to deal with a wide variety of people, and knowledge of programs. Education and Experience: Must be comfortable with computers, databases, Excel and Word. Starting amount is $11.75 per hour.
